Job Description

SUMMARYThe Aftercare Support Worker will facilitate follow up interventions with our existing cohorts and in other parts of our service who have graduated or completed programs offered by Nimkee Nupigawagan and who are wanting to make a positive lifestyle choice.The Aftercare Support Worker will facilitate wholistic aftercare services including referrals for participants who have completed treatment. This includes assessing with participants and the team- priority areas of focus in 9 different domains such as housing, relationships, spirituality and physical needs and working diligently to achieve participant and Nimkee goals.Aftercare services will be provided consistent with the philosophy, approach, goals and learning environment espoused by Nimkee Nupigawagan Healing Centre Inc.MAIN RESPONSIBILITIES:Under the direction of the Manager of Community Services- Mental Health and Addiction, the Aftercare Worker is responsible for:•Coordinating and providing aftercare activities and services to individuals whocomplete treatment at Nimkee Nupigawagan Treatment Centre or elsewhere;•Providing systems advocacy for people experiencing addictions issues;•Providing information sessions and support to community-based referrals•Providing education and information sessions to community members and supportfor community-based addictions programs;NIMKEE NUPIGAWAGAN HEALING CENTREAftercare Support Worker(One year Contract Employment Opportunity)•Networking with Family Support/Wellness Workers, Crisis Workers and HealthDirectors from the communities for Aftercare Programming needs;•Regular travelling in the Region will be a requirement;•Processing aftercare plans, networking with community services and working as anoutreach/liaison person to communities;•Entering information into the electronic database and providing reports to theManager of Community Services- Mental Health and Addiction•Participating effectively in the case review process with the treatment services teamand allied health and social professionals;•Accept all aftercare referrals in cooperation with Case Management Systems andsupervisor•Ensure that culture and traditions are integrated into the program activities

Experience and Qualifications

QUALIFICATIONS1.Education and Work Experience•A minimum of an HBSW is required along with experience in the addictions andmental health field or significant experience and working towards designation.•Experience in the field of alcoholism and drug addiction as it relates to theIndigenous people.•Indigenous person with lived experience preferred.•Experience in justice, child welfare, health or related fields•Experience working with Indigenous people and in facilitating programs•Extensive Administrative Experience.•Candidates without the specific education qualifications and where the positiondoes not require certified qualification to practice, but who possess an appropriatecombination of experience and other academic qualifications may be considered.
