PROGRAM AREA: HEALTH SERVICES
POSITION TITLE: COMMUNITY HEALTH NURSE – RN/RPN – HEALTH PROMOTION AND EDUCATION
REPORTING PROTOCOL: REPORTS TO THE COMMUNITY HEALTH MANAGER/DESIGNATE

POSITION SUMMARY:

In collaboration with health center staff, the community health nurse will develop and implement plans for improving the health status of community members through the delivery of educational programs. The community health nurse will provide programming with emphasis on health promotion, chronic disease prevention and management, maternal child health, school health, vaccine preventable disease, and other duties as delegated. This role plays a vital part in supporting holistic wellness and building strong relationships within the community.

Experience and Qualifications

EDUCATION/PROFESSIONAL REQUIREMENTS:
▪ Bachelor of Nursing Degree recommended
▪ Minimum 2-year diploma in Nursing with relevant experience may be considered based upon the merit of the candidate
▪ Current unrestricted registration with CNO
▪ RNAO or RPNAO membership
▪ BCLS Certification required
▪ Requirement of criminal record check/police information (within the past three months including, vulnerable sector search) prior to the first day of hire

SKILLS REQUIRED:
• Experience working in First Nation Health recommended
• Knowledge and understanding in health services and program delivery of health administration
• A minimum of two years recent, relevant experience and demonstrated competency in public health, primary care/primary health care, health promotion or community development preferred
• Relevant experience includes applying principles of health promotion, primary prevention, population health, primary health care, & community development in public health, primary care/primary health care
• Computer literacy, familiarity with Microsoft Office products (Outlook, Excel, Word and Access Programs; PS Suites an asset)
