Summary
Chippewas of the Thames First Nation is seeking a dedicated and experienced Community Health Nurse Supervisor to lead and support community-based health initiatives. This position plays a key role in delivering
high-quality, culturally relevant nursing services to individuals and families in our community. The successful candidate will provide leadership in planning, implementing, and evaluating health programs, with a strong emphasis on health promotion, disease prevention, and holistic wellness.

The CHN Supervisor will work in alignment with the Nation’s Comprehensive Community Plan and Community Story, while upholding the high standards required to maintain our accreditation through the Canadian
Accreditation Council.

Responsibilities
• Coordinate and deliver community health nursing programs through clinic visits, home visits, schools, and public education.
• Apply the nursing process to assess, plan, implement, and evaluate care for individuals and families.
• Lead annual clinics such as rabies vaccination and influenza immunizations.
• Update and implement the Community Pandemic Plan and develop a wellness action plan.
• Ensure compliance with Home and Community Care Program policies and Accreditation standards.
• Collaborate with internal and external stakeholders to address health needs and advance holistic care in the community.
• Participate in strategic initiatives to support the long-term vision and goals of COTTFN

Experience and Qualifications

Qualifications

  • Bachelor’s Degree in Nursing (BScN) from a recognized institution.
  • Registered and in good standing with the College of Nurses of Ontario.
  • Five (5) years’ experience in community health nursing.
  • Three (3) years of supervisory experience preferred.
  • Valid Ontario Driver’s License and access to a reliable vehicle.
  • Excellent communication, case management, and documentation skills.
  • Proficiency with Microsoft Office and Electronic Medical Records.
  • Training and certifications including:
    o First Aid & CPR
    o WHMIS
    o Mental Health First Aid
    o ASIST
    o Safe TALK
    o Cultural Diversity Training
    o Safe Food Handling
    o Privacy & Confidentiality
    o Immunization certification

ADDITIONAL ASSETS

  • First Nations Inuit Health (FNIH) approved training in Community Health
  • Knowledge of and sensitivity to Indigenous culture, language, traditions, and holistic health approaches
  • Experience working with First Nations communities
  • Familiarity with federal and provincial public health guidelines (e.g. PHAC, FNIHB, WHO)
  • Member of the Registered Nurses Association of Ontario (RNAO)
