Community Health Representative

GENERAL DESCRIPTION:

The Community Health Representative will work collaboratively to engage children, youth and adults in the promotion of physical, mental, emotional and spiritual wellness and the protection of health and prevention of illness and injury through the facilitation and/or coordination of a range of evidence-based primary and secondary health prevention and intervention programs and strategies to address risk and protective factors.

The CHR will also provide support and advocacy for Munsee-Delaware Nation individuals and families by facilitating and coordinating access to health services, addressing cultural and spiritual needs, networking with First Nation and Non-Aboriginal partners to ensure access to health and other services is a culturally safe experience for patients and their families.

KEY DUTIES AND RESPONSIBILITIES:

  1. Technical Functions:

Plan, facilitate and/or coordinate a range of evidence-based primary and secondary health prevention programs to individuals and/or groups, including but not limited to topics of disease prevention; personal hygiene practices; breast self-exam; personal safety; injury prevention; healthy lifestyles; preconception, pre- and post-natal health; immunization; HIV/AIDS; sexual health; environmental awareness; screening and management of diabetes and other chronic, degenerative, and common diseases.

  1. Administrative Functions:

Plan, implement and evaluate a variety of community-based and culturally appropriate campaigns to promote health and well-being and prevent disease. Create and maintain client files.

  1. Communication & Liaison

Work collaboratively in planning and implementation of health clinics including but not limited to diabetes, arthritis, and wellness clinics across the lifespan.

  1. Other Related Duties:
  • Performs other job-related duties as may reasonably be required by the Nation Manager.

QUALIFICATIONS:

  1. Preference will be given to candidates with Registered Practical Nurse diploma from a recognized post-secondary institution, current registration and in good standing with the College of Nurses of Ontario and current membership with the Registered Nurses Association of Ontario.
  2. Post-secondary certification as a Community Health Representative will be considered.
  3. Preference will be given to qualified individuals who are also certified as Diabetes Lay Educators.
  4. Minimum of three (3) years’ experience in health education/promotion.
  5. Knowledge of and respect for Lenape values, teachings, culture, and traditions is of critical importance.
  6. The ability to understand and speak the Lenape language will be considered a definite asset.
  7. Proficient in Microsoft Word, Excel, PowerPoint, internet and email software/s, and use of on-line learning resources.
  8. Must possess strong interpersonal skills.
  9. Must be able to work with minimum supervision.
  10. Must be able to work flexible hours, including evenings and week-ends.
  11. Must have valid driver’s license and access to a vehicle for on-the-job use.
  12. Candidate to provide vulnerable sector CPIC.
