Summary
Chippewa of the Thames First Nation is seeking a compassionate and knowledgeable Diabetes Educator to join the Health Department team. The successful candidate will deliver diabetes education and community
health programs to support and improve the wellness of community members. This role focuses on individual and group education, prevention programming, and holistic approaches that incorporate traditional and
cultural teachings.
Responsibilities
Client Support & Education:
• Assess, monitor, and support clients with diabetes in setting and achieving their health goals
• Provide one-on-one and group education on nutrition, physical activity, blood sugar monitoring, and diabetes management
• Conduct home visits (with approval) and school-based screenings and education
Program Delivery:
• Plan, deliver, and evaluate diabetes education programs and community initiatives
• Coordinate monthly foot care clinics and follow-up care
• Create and distribute educational materials via various media platforms
• Develop and offer workshops on healthy eating, traditional foods, and food safety
Community Engagement & Outreach:
• Collaborate with schools, health professionals, and other stakeholders to support clients
• Provide community-based diabetes education displays and information sessions
• Integrate traditional teachings into all health initiatives
Administration & Reporting:
• Research trends and service gaps in diabetes care
• Maintain accurate client records and report program outcomes
• Participate in team meetings and contribute to strategic planning
Qualifications
Education & Certification:
• Diploma in Diabetes Education, Indigenous Community Diabetes Support, or Certified Diabetes Educator (CDE) designation
• Diploma in Native Community Care or equivalent
• Additional certifications in foot care, Mental Health First Aid, ASIST, and First Aid/CPR are assets
• Minimum two (2) years’ experience working in a First Nation community
• Experience in diabetes education, community programming, and client counseling
• Familiarity with First Nation health service delivery and external service providers
Additional Requirements
• Valid Class G Ontario Driver’s License with access to a reliable vehicle
• $1M vehicle insurance coverage
• Clear Police Record Check with Vulnerable Sector Screening
• Current immunizations, including three (3) doses of the COVID-19 vaccine
• Flexibility to work evenings and weekends as needed
Conditions of Employment
• Must provide a satisfactory Vulnerable Sector Check upon hire and maintain it annually.
• Must sign and abide by confidentiality and code of conduct agreements.
• Flexibility to work evenings, weekends, and travel as required.
