Position:              Home and Community Care Nurse                                          

Term:                   Permanent Full-Time

Department:       Home and Community Care           

Division:               Health Centre                   

Salary:                 Competitive salary range

Hours:                  35 hours per week, Monday to Friday, 8:30a.m. to 4:30p.m.

JOB SUMMARY:  

The Home and Community Care (HCC) Nurse provides care and case management to the clients of the Home and Community Care program. The HCC Nurse will provide primary care within their scope of practice and participates in the development, implementation, monitoring and evaluation of services for individuals and families.

The HCC Nurse provides health education and promotion for client and family members in accordance with the standards of nursing practice.

Experience and Qualifications

EDUCATION:

  1. College Diploma in Registered Practical Nursing (RPN).
  2. Current member, in good standing, with the College of Nurses of Ontario.

EXPERIENCE:

  1. A minimum of two (2) years employment experience in nursing, including in a primary care or community setting.
  2. Experience working in First Nation communities is considered an asset.
