Summary:
The Home Support Worker (HSW) provides in-home support, care, and assistance to clients of the Home and Community Care Program. The HSW supports routine activities of daily living to help clients remain as healthy and independent as possible while maintaining dignity, compassion, and cultural sensitivity.
The HSW delivers services based on individual care plans and in accordance with relevant program policies and procedures, applicable legislation, accreditation requirements, and Chippewas of the Thames First Nation (COTTFN) policies. The role works in collaboration with the broader health care team and supports the vision of the Chippewa Health Centre and COTTFN.
