JOB SUMMARY:
The Regional Renal Program oversees the delivery of renal care services across the Erie St. Clair (ESC) region. The Ontario Renal Network’s (ORN) is committed to supporting the unique needs of Indigenous peoples and developed a strategic initiative as part of the Ontario Renal Plan (ORP) 3 to develop a strategy in partnership with First Nations, Inuit, Métis, and Urban Indigenous (FNIMuI) people to improve their kidney care. The ORN developed an Indigenous Chronic Kidney Disease (CKD) Care Lead role to assist Renal Programs in achieving their goal to improve kidney care in these communities. The Indigenous CKD Care Lead supports the work of the ESC Regional Renal Program’s engagement with the FNIMuI communities and partners to develop and implement quality initiatives focused on ensuring care is culturally safe and address the unique needs of the Indigenous people within the region. As a vital team member, the Indigenous CKD Care Lead role will provide support and advocacy for Indigenous patients, families and caregivers by facilitating and coordinating access to renal services, addressing cultural/spiritual needs and building relationships with Indigenous and non-Indigenous community and healthcare partners. This position will assist in bridging the service gaps between Indigenous communities and health care providers by facilitating linkages to services that may include (but are not limited to) Kidney Disease, mental health and addictions, other chronic disease management, primary care and acute care. The Indigenous CKD Care Lead will support the strategic goals and initiatives of the Windsor Regional Renal Program, Ontario Renal Network’s – Ontario Renal Plan (ORP) and partnership with the neighbouring Regional Renal Programs.
RESPONSIBILITIES:
1) Collaboration, partnerships and knowledge exchange
- Establish and maintain relationships with regional FNIMuI and non-FNIMuI partners
- Communicate regularly with Windsor Regional Renal Program leadership team, regional partners and relevant disciplines on FNIMuI provincial team
- Provide support and expert consultation to healthcare providers within the region to facilitate access to care, services and resources.
- Provide regional representation in other relevant provincial quality initiatives at committees such as the Ontario Renal Network’s CKD Screening Community of Practice and Windsor Regional Hospital Indigenous Committee
- Work with other entities in the region that are providing renal care services, to ensure an integrated patient-centered regional approach to palliative care services that is appropriate to FNIMuI patients
2) FNIMuI supportive and palliative care services
- Provide support to FNIMuI patients with chronic kidney disease and their families by facilitating access and coordination of services both within and beyond the region
- Initiate referral to appropriate personnel based on knowledge of the scopes of practice of other members of the health care team/ advocates on behalf of patients and family/significant others
- In consultation with the patient, family/significant others and the health care team, individualizes the multidisciplinary patient care pathway to reflect interventions which meet identified needs while promoting cultural appropriateness, safety, privacy, and confidentiality for patient/family/ significant others
- Review current referral processes and recommend appropriate ways to increase access to renal services
- Support regional and provincial initiatives to improve the detection and prevention of CKD and is risk factors for FNIMuI people
- Develop a regional inventory of renal care related programs and services for Indigenous patients and families
- Monitor and record types of services provided for Indigenous patients and families at the hospital and in the community (if applicable through existing reporting mechanism; ie. workload measurement and EMR – Cerner)
- Document the experiences of FNIMuI renal patients and their families, in collaboration with the WRRP Leadership team and develop a work plan with key targets and timelines to address support issues, cultural needs and networking with FNIMuI communities
- Support and participate in regional and provincial palliative care research and innovation projects
- Support continuing professional development programs in FNIMuI care for health professionals in the renal program and regionally
- Support provincial and regional program evaluations by providing input into the FNIMuI supportive and palliative strategic priorities, share best practices, and performance management
3) Operations
- Participates in the development of the annual RRP work plan identifying key activities, deliverables, performance measures and timelines in collaboration with the RRP Leadership tam and Ontario Renal Network
- Completes and submits the Engagement with Indigenous Partners Status Update document as per reporting requirements with the Ontario Renal Network
- Participates as an active member of the WRH Renal Program leadership team
