The successful applicant will also possess effective communication and interpersonal skills, will have the ability to work independently and as part of a team, and will have strong attention to detail and accuracy.

Each member of Bluewater Health is called to embrace patient and family-centred care in its fullest form. This includes using techniques and strategies to communicate “I care and I respect you,” providing information in a way patients and families can understand, listening to patients and families to learn what an exemplary healthcare experience means to them, and seeking their perspective throughout their stay and during improvement initiatives.

Principal Duties:

– Engage with patients early through Indigenous identification, care team and/or community referrals.
– Provide support, information and advocacy for the patient while attending the Emergency Department
– In consultation with the patient, family, and health care team, individualize the care path to reflect interventions to meet identified needs and goals while promoting cultural appropriateness, safety, privacy and confidentiality for patients
– Develop a patient centred plan of care that incorporates traditional healing/culturally safe practices, community connection, and preventative health services based on client choice
– Facilitate individual and group education sessions, assist in health promotion events and preventative health events
– Collaborate with other Indigenous Navigators
– Support discharge plans with recommendations to enhance patient care working in collaboration and in partnership with hospital health care teams, and the patient’s circle of care
– Maintain collaborative working relationships with other members of the health care team
– Liaise with community partners as it relates to effective client care
– Support patients and their families to understand the plan of care in hospital and community-based services
– Liaise with NIHB and local community services for any equipment, medication or transportation needs
– Provide support and consultation to healthcare providers to facilitate access to care, services or resources for Indigenous patients, their families and caregivers
– Support the building of relationships between hospital, community partners and Indigenous communities based on mutual trust, respect, open communication, creative thinking, and cohesive team effort
– Foster an environment of cultural safety for all patients, caregivers, and their families.
– Attends and supports multidisciplinary rounds, clinical and staff meetings as needed
– Support communication between patient/families and various members of the health care team, community services, Traditional Healers, Elders, and Knowledge Keepers
– Maintain documentation of patient interactions: assessments, plans and actions in the electronic medical record system and prevention plan records

Experience and Qualifications

Qualifications:

– Member in good standing with a current Certificate of Registration with a Regulated Health Professions College in Ontario preferred
– Nursing or social work/social service background preferred, other allied health professionals will be considered
– Certificate or diploma in Addictions preferred
– Identifies as Indigenous (First Nation, Metis, Inuit) with lived experience
– Indigenous-based Trauma Informed education preferred
– Preference will be given to someone with experience working with a First Nation(s) community

Skill, Ability and Experience:

– Previous experience working in Indigenous health; established relationships with Indigenous community health services, local hospitals, regional/local health centres, and community based health services, would be an asset.
– Knowledge of case management/system navigation and community resources – particularly Indigenous specific
– Excellent understanding of Indigenous communities, regional/local health centres and community based health services an asset
– Experience with and knowledge of local/regional Indigenous language, cultures, customs, traditions required
– Demonstrated ability to work with Indigenous Elders, Traditional Healers, and Knowledge Keepers required
– Possesses well-developed interpersonal and relationship management skills, supported by sound critical thinking, problem-solving and conflict resolution skills to interact effectively and build respectful, trusting and collaborative relationships required
– Proficiency using the MS Office Suite of products, including Outlook, Word, PowerPoint and Excel required
– Knowledge of Non-Insured Health Benefits (NIHB) preferred
– Valid driver’s license and access to reliable transportation.
