The Indigenous Transitions Navigator or bami’iwe (Ojibwe for s/he takes care of people, helps people, gives care) position will support CKHA in its commitment to reconciliation. The role has been developed from guidance from the community and local Indigenous partnerships.
ESSENTIAL QUALIFICATIONS (Knowledge, Skill, Experience)
-Minimum of five years’ work experience in an Indigenous health and or social service organization, hospital or community-based case management role
-Knowledge of biopsychosocial assessment models, and social determinates of health
-Nursing or social work/social service background (preferred)
-Experience working with First Nations community and urban indigenous community
-Preference will be given to an Indigenous person from a local indigenous community
-A member in good standing with current registration with a regulated professional college (preferred)
ESSENTIAL COMPETENCIES (Personal Skills and Attributes)
-The role will work in partnership and walk beside Indigenous patients, using a two-eyed seeing approach to provide care. The Navigator supports planned transitions in care for Indigenous patients
-This role will facilitate culturally safe and relevant transitions in care, for patients returning home to a First Nation or urban setting in conjunction with the healthcare team.
-Support and advocate for Indigenous patients, caregivers, and their families by appropriately responding to their health needs and goals. The primary aim of this position is to improve patient experience, pathways and transitions within the hospital and from hospital to home or community care in a cultural relevant manner
ACCOUNTABILITIES (Major Responsibilities of the Position)
Case Management and System Navigation
-Awareness of Indigenous practices and teachings in regards to healthcare/service navigation and awareness that the system is in reference to the holistic needs of indigenous patients
-Engage early in patient stay to support care and support care transitions, and discharge planning
– Support coordinated transitions and discharge planning with recommendations to enhance patient care plans working in collaboration and in partnership with hospital health care teams, and the patient’s circle of care, including palliative care teams and cancer care navigators.
-In consultation with the patient, family, and health care team, individualize the care path to reflect interventions to meet identified needs and goals while promoting cultural appropriateness, safety, privacy and confidentiality for patients
-Support patients and their families to understand the plan of care and hospital and community-based services.
-Complete assessments, consultations and referrals based on client and family needs, considering Indigenous determinants of health
Education and Promotion
-Provide informal educational opportunities to the care team in harmony with local Indigenous beliefs, through coaching and teaching to support new learnings.
-Meaningfully consult and assist in implementing the advice and directions provided by community health and Indigenous leaders and knowledge keepers
-Consult with and/or partner with community service agencies to avoid service/programming duplication and ensure service plan supports complement community initiatives underway or being planned
Administrative
– With the Executive Lead, EDIAR participate in planning sessions with hospital leadership (as required) to ensure all planning maintains cultural integration
-Assist the Executive Lead, EDIAR in the development of culturally appropriate care standards, policies, procedures, practices, and hospital spaces to better support the health care needs of Indigenous patients, caregivers, and their families
-Assist the Executive Lead, EDIAR with presentations (as required)
-Assist with data collection, patient trend information, service utilization rates to determine and assess service gaps and needs
Quality Improvement
-Utilize patient and community feedback to improve quality of care, services and patient experience
-Establish, monitor and maintain appropriate data for quality improvement indicators, as required
-Assist in the design and development of tools for data collection as part of service evaluation and monitoring for quality improvement
