Canada Summer Jobs- $16.55/HR 35 HR/week for 8 weeksCase Management Social Worker- Job DescriptionJob Summary:The Case Management Social Worker will work closely with clients to assess their needs, develop action plans, and connect them with necessary community resources. This rolerequires a compassionate and dedicated individual capable of both individual client work and collaboration within an interdisciplinary team.Key Responsibilities:● Conduct individual assessments to determine client needs and service requirements.● Develop and implement personalized action plans for clients.● Refer clients to necessary community supports such as housing, legal aid, and medical services.● Monitor and evaluate client progress.● Advocate on behalf of clients as needed.● Facilitate partnerships with community organizations.● Provide one-to-one support to help clients navigate their employment and personal challenges.● Assist in the development and planning of programs and workshops for client assistance.● Maintain accurate client records, ensuring sensitive information is stored securely.● Conduct follow-ups, attend case conferences, and participate in team meetings.● Prepare regular reports on client progress and service efficacy.Skills Development:● Adaptability: Learn to adjust plans and behaviors in response to changing circumstances.● Collaboration: Enhance the ability to support and contribute to team efforts.● Communication: Develop skills in sharing and receiving information effectively.● Creativity and Innovation: Cultivate the capacity to think outside the box and introduce new ideas.● Digital Skills: Gain proficiency in using digital tools for information management.● Numeracy: Improve abilities in handling numerical data effectively.● Problem-solving: Enhance analytical skills and decision-making capabilities.● Technical Skills: Acquire practical skills related to the field of social work and case management.● Other Skills: Build on cultural and diversity sensitivity, leadership abilities.Required Language:EnglishSupervision and Mentoring:This position will be directly supervised by Senior Career Staff, receiving both guidance and mentorship throughout the placement.
