Summary

The Chippewas of the Thames First Nation Health Department is seeking a compassionate and dedicated Community Health Nurse (CHN) to deliver high-quality community health services. The CHN will work collaboratively to identify, assess, plan, implement, and evaluate public health programs addressing the physical, emotional, and spiritual health of community members. This position plays a critical role in supporting culturally safe and holistic care in alignment with the 14 Social Determinants of Health, the COTTFN Comprehensive Community Plan, and the Chippewa Community Story.

Responsibilities

  • Provide direct nursing care through home visits, clinic services, school visits, and workshops.
  • Deliver and support programs in communicable disease control, maternal/infant care, chronic illness management, and school/elder care.
  • Plan and implement public health campaigns, wellness education, and immunization programs.
  • Conduct community assessments and develop action plans for health promotion.
  • Ensure compliance with federal and provincial health regulations and standards.
  • Collaborate with internal departments, local health services, and regional/federal health agencies.
  • Maintain accurate records, submit required reports, and participate in ongoing program evaluation.
  • Participate in emergency planning, environmental health assessments, and outbreak management.
  • Respect and integrate traditional teachings and cultural practices into service delivery.

Experience and Qualifications

Qualifications

  • Bachelor of Science in Nursing (BScN) or equivalent from a recognized institution.
  • Registered and in good standing with the College of Nurses of Ontario.
  • Minimum 3 years’ experience in community health nursing.
  • Training approved by First Nations and Inuit Health Branch (FNIHB).
  • Current First Aid/CPR, WHMIS, and COVID-19 vaccinations (3 doses).
  • Proficient with Microsoft Office and Electronic Medical Records (EMR).
  • Additional certifications in:
    • Mental Health First Aid
    • Applied Suicide Intervention Skills Training (ASIST)
    • Cultural Diversity and Privacy Training
    • Safe Food Handling and Safety Talks
  • Valid driver’s license, insurance, reliable transportation, and a clear Vulnerable Sector Check.
  • Knowledge and appreciation of Indigenous culture and the health-related needs of COTTFN.
  • Preference will be given to qualified applicants who are members of a First Nation as per Section 16(1) of the Human Rights Act.

ADDITIONAL ASSETS

  • First Nations Inuit Health (FNIH) approved training in Community Health
  • Knowledge of and sensitivity to Indigenous culture, language, traditions, and holistic health approaches
  • Experience working with First Nations communities
  • Familiarity with federal and provincial public health guidelines (e.g. PHAC, FNIHB, WHO)
  • Member of the Registered Nurses Association of Ontario (RNAO)

CONDITIONS OF EMPLOYMENT

  • Must provide a satisfactory Vulnerable Sector Check upon hire and maintain it annually.
  • Must sign and abide by confidentiality and code of conduct agreements.
  • Flexibility to work evenings, weekends, and travel as required.
