POSITION: INDIGENOUS HEALTHY BABIES HEALTHY CHILDREN PROGRAM COORDINATOR

TERM: PERMANENT 35 HOURS/ WEEK

Wage: BASED ON EXPERIENCE AND QUALIFICATIONS

PROGRAM DESCRIPTION:
Under the supervision of the Child and Youth Section Manager, the Indigenous Healthy Babies Healthy Children (IHBHC) Program Coordinator provides culturally grounded supports to urban Indigenous
families during the prenatal period and for children from birth to six years of age. The Coordinator delivers family-centered services that promote healthy pregnancies, early childhood development, positive parenting, and overall family well-being.

Duties:
• Plan, design, implement, and evaluate culturally grounded programming for Indigenous families from the prenatal period to children six years of age
• Provide in-home and community-based supports, education, and one-on-one guidance to families to promote healthy pregnancies, child development, and positive parenting
• Deliver parenting education, prenatal/postnatal information sessions, workshops, and group programming, including certificates of completion where applicable
• Conduct client intakes, assessments, goal planning, and ongoing case management to identify family strengths, needs, and risk factors
• Provide referrals and service coordinator to connect families with medical, social, developmental and cultural supports
• Actively promote the program through outreach activities, community engagement, and the development of promotional materials (pamphlets, calendars, and flyers)
• Build and maintain strong working relationships with hospitals, public health, early years services, community agencies, and Indigenous service providers
• Coordinate external partners and ensure services are delivered in a culturally safe, family-centered and holistic manner
• Maintain accurate clients files, case notes, and data, ensuring confidentiality and completion of required monthly, quarterly, and annual reports
• Participate in community events, gatherings and outreach activities on an ongoing basis
• Work flexible hours, including evenings and weekends as required
• Attend training sessions and workshops as identified to enhance skills and service delivery
• Communicate effectively and professionally with clients, staff, and community partners
• Perform other related duties as assigned by the Child and Youth Section Manager

Experience and Qualifications

Qualifications:
• Post-Secondary education in Health, Social Services, Early Childhood Development, or a related field; or a combination of education and five (5) years of related experience
• Knowledge and awareness of Indigenous culture and history, child welfare, and the impacts of intergenerational trauma and residential schools
• Experience in program planning, development, implementation, and evaluation
• Demonstrated understanding of prenatal, maternal, infant, and early childhood development, including healthy pregnancies, infant care, nutrition and family well-being
• Experience in providing in-home and community-based family supports, including early intervention, service coordination, referrals, and navigation of health and social service systems
• Demonstrated initiative with strong planning, organizational skills, and the ability to meet deadlines with minimum supervision
• Ability to work independently and collaboratively within a team environment
• Knowledge of available Indigenous service resources in the community and surrounding area
• Excellent oral and written communication, interpersonal, and counselling skills
• Proven, positive, proactive experience in working with Indigenous families
• Proficiency in Microsoft Word, Outlook and database programs
• Valid First Aid/CPR and Safe Food Handling certifications, or willingness to obtain
• Valid driver’s license and access to a reliable vehicle
• Ability to work under pressure, meet deadlines, and work flexible hours
